Contact Information Page

You can create, view , or update your contact information—including your address or addresses, package delivery information, and phone numbers—on this page.

The Street Address 1, City, State, Zip Code, Address Type, and Daytime Phone fields are required. Information must be entered in each in order for the page to be saved. If you fail to enter valid information in a required field, a popup error message is displayed when you click Save or Next.

If you have a PO Box as your mail address, you must also provide a second address (and assign it an Address Type of Federal Express address) so that materials sent to you via a courier service can be delivered there (courier services cannot deliver to PO Box addresses).

Note: One, and only one, address must be selected as the Preferred Address, as indicated by a dot in the Preferred Address radio button. This address will be used first to contact you. If you provide only one address, it will automatically be selected as the Preferred Address. If you delete the Preferred Address, the earliest entered of the remaining addresses will be automatically selected as the Preferred Address.

The Contact Information page can be accessed only from the Personal Profile page. When creating a new peer review profile, open that page, enter the required information, and click Next. The Contact Information page appears.

Enter your Street Address.
(Optional) Enter additional address information (such as a suite number or PO Box) in Street Address 2.
Enter your City.
Using the dropdown menu, specify your State or territory.
Enter your Zip Code, entering five digits in the first field, and either four or zero digits in the second field.
Using the dropdown menu, specify this Address Type.
(Optional) Using the dropdown menu, specify this Address Location.

Note: In the Address Type field:

select Mail for the address to which correspondence should be delivered during the week

select Saturday to indicate where you want correspondence to be sent on a weekend, if it is different than your Mail address.

If this address is the Preferred Address, click in the radio button. If this is the only address you have entered, you will not be able to change this status.

(Optional) Click in the checkbox if this address is appropriate for Overnight Delivery.
Enter your ten-digit Daytime Phone. An extension number is optional.
(Optional) Enter your ten-digit Evening Phone, Fax, and/or Cell numbers.
Click next. Your personal profile is saved, and the Contact Information page opens.
