This tab allows you to create performance measures for all the grant activities you intend to measure.
Applicant Type
· Begin by selecting “Applicant Type.” If you are creating/revising performance measures as an applicant or existing SIF grantee, select “Grantee.” If you are an existing SIF grantee and want to create or revise performance measures for a subgrantee, select “Subgrantee.”
Applicant Name
· Select applicant/grantee or subgrantee name from drop down list.
· If you are an existing SIF grantee and want to add a new subgrantee to the drop-down list, enter subgrantee name in textbox and select Start Date then click "Add Subgrantee" button.
· If you want to disable a subgrantee, select the subgrantee name from the Applicant Name drop-down list and End Date then click "Disable Subgrantee" button.
· If you want to enable a subgrantee that is disabled, select subgrantee name from the Disable Subgrantees list and click the "Enable Subgrantee" button.
Problem Statement
· Briefly describe the problem that you seek to address through your SIF grant activities.
· If you are an existing SIF grantee, please refer to the SIF performance measures guidance for instructions on completing this information for subgrantees.
Level of Evidence
· For applicants or grantees, select the existing and target levels of evidence of effectiveness for your program model.
Select an evaluation instrument for your evaluation and provide a description of the instrument. If your instrument is not listed, select “Other” and then provide a description.
· If you are an existing SIF grantee, please refer to the SIF performance measures guidance for instructions on completing this information for subgrantees.
Outputs, Outcomes, End Outcomes, and Focua Area End Outcomes
· The SIF has both mandatory and optional measures. Mandatory SIF measures are designated by an asterisk (*). Please see the SIF performance measures guidance for descriptions of each measure and instructions on how to count/report these measures. If you are an existing SIF grantee, please refer to the SIF performance measures guidance for instructions on completing this information for subgrantees.
· Select the outputs and outcomes your program will address. For each measure, select and describe the specific instrument(s) you will use to measure the output or outcome. Include the title of the instrument(s), a brief description of what it measures and how it will be administered, and details about its reliability and validity if applicable.
· Enter the target number for your output or outcome. Targets must be whole numbers, not percentages.
· Where applicable, select an appropriate focus area and baseline. Baselines must be whole numbers, not percentages.
· If you plan to serve beneficiaries in more than one focus area, select “Add Another G3-3.18 End Outcome” and choose another focus area.
· To create additional measures, select “Add Another FA End Outcome” and select desired focus areas, outputs, and outcomes, as appropriate.
Interventions
· Interventions are the program activities that you will carry out to address the problem identified in the application. Only interventions that correspond with the focus area(s) you selected will appear.
· Select all interventions that are a part of your program design. Clicking “Add User Intervention” will allow you to add and describe any intervention not listed.
After entering all information, select, “Complete PM.” This will return you to the Home Page.
If you are an existing SIF grantee and wish to create performance measures for another subgrantee, repeat the process. If you would like to continue to the next step of the module, click “Next.”
